Three Year Quality Enhancement Reports

Every three years, the Butler County Board of MRDD is mandated by the Ohio Department of MRDD to complete a Three Year Quality Enhancement Written Report on all individuals receiving waiver services and supported living services. The person receiving services and his/her guardian/family members are interviewed concerning satisfaction with the delivery of services, as well as health and safety concerns.

The reports are organized by quarter. Currently the second, third and forth quarter of 2008 is available and also the first quarter of 2009.

For the second, third and forth quarter reports for 2008, five questions were asked of individuals and their family/advocate or guardian and five questions were asked of the family/advocate or guardian.

The individuals were asked:

• Do you like where you go/work during the day?
• Do you like your home staff?
• Do you feel safe in your home?
• Do you feel safe where you go/work during the day?
• Do you feel happy when you are at home?

The family/advocate/guardians were asked:

• How satisfied are you that the staff are providing the services as identified in the ISP?
• Overall do you believe the person is satisfied with services being provided by their day program?
• How satisfied are you with the individual's current residential provider?
• How satisfied are you with the individual's direct care staff?
• Do you believe the person's home is safe and secure?

The answers to those questions are provided in the following PDF documents. The answers are organized by question, followed by an alphabetical listing of the providers. Not all providers will have responses because not all of the providers were surveyed.

• How satisfied are you with the provider?
• How satisfied are you with the current direct care staff?
• Do you feel safe where you live/work?

The family/advocate/guardians were asked:

• How satisfied are you with the services being provided?
• Do you feel the person likes where he/she lives/works/attends?
• How satisfied are you with the provider?
• How satisfied are you with the current direct care staff?
• Do you feel the person feels safe where he/she lives/works/attends?
