Incident reporting is a resource for people concerned about the well-being of individuals with mental retardation and developmental disabilities.  Incident reporting not only keeps the agency informed, but in some cases reports also are filed on a state level.  Click here to see the Rights of Individuals with Disabilities.

What Is Abuse?

Abuse is classified in three ways:  physical, sexual and deprivation of real or personal property.  Click here for physical and behavioral indicators of abuse and neglect for persons with MRDD.

Physical abuse is one person causing physical harm to another.
Sexual abuse is unwanted touching, non-consensual sexual acts.
Verbal abuse is using words or gestures to threaten, coerce, intimidate, harass or humiliate another individual.
Mental abuse is harassing, humiliating and invoking fear to another individual.
Violation of Rights is any incident where basic rights are ignored.
Exploitation is using another individual's money or belongings for personal benefit, profit or gain.

What Is Neglect?

Neglect is failing to provide treatment, care, goods, supervision, or services necessary to maintain health and safety.

What Is An MUI?

A Major Unusual Incident (MUI) is any of the following:

Death
Attempted Suicide
Abuse
Neglect
Any act involving an individual with mental retardation or a pattern of activities that requires the involvement of Law Enforcement, resulting in an arrest, incarceration or charges of the individual.
Relocation of an individual or inability to provide services for a 24 hour period.
A life threatening reaction resulting from medication or failure to take medication or to follow prescribed dietary and medical treatment plans, or a pattern of medication errors.
An injury or illness that results in unanticipated acute hospitalization or treatment in a similar facility.
Any violation of rights pursuant to Ohio Revised Code Section 5123.62 effecting health and safety.

If a worker or volunteer in MRDD services observes or has reason to believe that a major unusual incident involving a person with a developmental disability has occurred, that person must report the occurrence to the Office of Incident Review.  Such a report must be made within 24 hours of the occurrence and a written report about the occurrence must be made to the ODMRDD Incident Tracking System of the Office of Incident Review within 24 hours.

What Is A UI?

An Unusual Incident (UI) is an event or occurrence not consistent with routine operation, policies, procedures or the care and habilitation plan of an individual with MRDD, but is not abuse, neglect or a major unusual incident. 

All licensed or certified service providers shall record the occurrence of UI's and make such records available to the Butler County Board of MRDD upon request.  When an Unusual Incident occurs while an individual is receiving services from the Butler County MRDD Programs, the county board shall record the incident and make the records available upon request to the individual or legal guardian.

UI's are recorded using the internal procedures of the agency with which the observing worker is affiliated.  Unusual incidents need not be reported to the Office of Incident Review.

What To Report

It is important to report any suspicions that an individual with mental retardation and/or developmental disabilities is being mistreated.   These reports include suspected abuse, neglect or any major unusual incidents (MUI's).  Click here to see more about what to report.
